Double C-Loop Haptic Lenses Are Rotationally Stable – A Systematic Review

The purpose of this review is to summarize the clinical rotational stability outcomes reported in patients following implantation of the POD platform with double C-loop haptics. A literature search in PubMed (US National Library of Medicine) was carried out to find publications, both prospective and retrospective, which report rotational stability outcomes in patients who were implanted with this platform after cataract or refractive lens exchange surgeries. This platform is used in different commercially available intraocular lenses (IOLs), both monofocal and trifocal, made of hydrophilic or hydrophobic material. 19 clinical peer-reviewed studies published between 2013 and 2024 were included in this review. The data reported in the clinical publications was analyzed in detail, focusing on rotational stability (mean rotation values and ranges, and percentage of eyes with some degrees of rotation), measurement methods and rotation follow-up period. Our review encompassed a total of 1428 eyes implanted with the POD platform analyzed at different follow-up periods, up to a maximum of 26 months. The mean rotation across all the studies was 2.61 °.The analysis carried out in this review leads us to conclude that the POD platform provides good rotational stability when used with different IOL models based on monofocal or trifocal designs and made using hydrophilic or hydrophobic material.

Long-term Clinical Outcomes After Bilateral Implantation of Two Trifocal Diffractive IOLs

Purpose: To show clinical outcomes after bilateral implantation of either a hydrophobic FineVision POD F GF or a hydro-philic FineVision POD F intraocular lens (IOL) (Beaver-Visitec International, Inc).

Methods: A total of 110 patients with cataract were randomized to receive either POD F GF or POD F IOLs and followed up for 2 years. The measurements included refraction, monocular and binocular uncorrected and corrected distance (UDVA and CDVA), intermediate (UIVA and DCIVA), and near (UNVA and DCNVA) visual acuities, defocus curve, photopic and mesopic contrast sensitivity, photic phenomena, and patient-reported outcomes.

Results: The mean spherical equivalent was similar in both groups and stable across visits (< 0.25 diopters [D]). At 2 years, 81.8% and 90.5% of eyes were within ±0.50 D in the POD F GF and POD F IOL groups, respectively (100% for ±1.00 D in both groups). At 2 years, 100% and 90.5% of the patients presented a binocular CDVA of 20/25 or better; 93.9% and 85.7% of patients a DCIVA of 20/25 or better and 87.5% and 70% of patients a DCNVA of 20/25 or better, for the POD F GF and POD F IOL groups, respectively. The defocus curve showed continuous visual acuity, being 20/32 or better in both groups over a 4.50 D range. Both groups presented good contrast sensitivity for photopic and mesopic conditions, and the size and intensity of halo and glare phenomena were similar between the two. Both groups also included a high percentage of patients who reported that they had stopped wearing glasses and greater than 80% satisfaction and recommendation levels.

Conclusions: This study showed comparable long-term visual and refractive outcomes of both IOL models after cataract surgery.

Long-term trifocal toric intraocular lens outcomes in Asian eyes after cataract surgery

Purpose: To determine the visual and refractive outcomes of trifocal toric intraocular lens (IOL) implantation in Asian eyes with cataract and astigmatism.

Setting: Asian Eye Institute, Makati City, Philippines.

Design: Prospective study.

Methods: 187 eyes were implanted with the FineVision POD FT IOL and followed for 2 years. The measurements included subjective refraction, uncorrected distance visual acuity (UDVA), corrected distance visual acuity (CDVA), uncorrected intermediate visual acuity (UIVA), distance-corrected intermediate visual acuity (DCIVA), uncorrected near visual acuity (UNVA), distance-corrected near visual acuity (DCNVA), defocus curve, photopic and mesopic contrast sensitivity, and rotational stability.

Results: The mean spherical equivalent and cylinder values were, respectively, -0.07 ± 0.34 diopters (D) and -0.48 ± 0.31 D at the 2-year follow-up. At this same follow-up, 69.4% of patients had monocular CDVA and 79.3% had binocular UDVA ≤0.0 logMAR. For intermediate vision, monocular DCIVA reached 74.1%, while binocular UIVA was 86.2% ≤ 0.1 logMAR. Near vision showed 63% for monocular DCNVA and 70.7% for binocular UNVA ≤0.1 logMAR. The mean values at this follow-up were 0.02 ± 0.08, 0.08 ± 0.10, and 0.10 ± 0.12 logMAR for CDVA, DCIVA, and DCNVA, respectively. At the visual acuity threshold of ≤0.20 logMAR, the binocular defocus curve extended up to -3.75 D. Stable contrast sensitivity values were obtained at the different follow-ups. The mean IOL rotation was ≤2 degrees.

Conclusions: The FineVision POD FT IOL yielded good visual outcomes at far, near, and intermediate distances with accurate refractive outcomes and good rotational stability in Asian eyes.

One-Year Clinical Outcomes Following Aspheric Toric Monofocal IOL with a Double C-Loop Haptic Design Implantation

Purpose: This study aimed to evaluate one-year clinical outcomes in cataract patients with pre-existing corneal astigmatism implanted with a biconvex aspheric toric monofocal intraocular lens (IOL) with a double C-loop haptic-design.
Methods: One hundred and eighteen patients (236 eyes) with corneal astigmatism (≥ 0.75D) were implanted bilaterally with the PODEYE toric IOL and assessed up to 1-year after surgery. Postoperative evaluation included monocular and binocular uncorrected distance visual acuity (UDVA) and corrected distance visual acuity (CDVA), under both photopic and mesopic lighting conditions, refraction, photopic and mesopic contrast sensitivity (with and without glare), and rotational stability.
Results: At the last postoperative visit, 74.6% and 95.8% of eyes were within ± 0.50D and ± 1.00D of the target refraction, respectively. About 78.0% and 97.5% of eyes showed a postoperative refractive cylinder of ≤ 0.50D and ≤ 1.00D, respectively. The mean manifest spherical equivalent value was 0.16± 0.40D, and the mean refractive cylinder value was − 0.42± 0.33D. 97.5% and 100% of patients had a binocular UDVA and CDVA of ≥ 20/32, respectively. The mean binocular UDVA and CDVA were 0.01± 0.09 and − 0.03± 0.07 logMAR, respectively. Under mesopic conditions, 79.5% and 89.8% of patients showed a binocular UDVA and CDVA of ≥ 20/32, respectively. The mean binocular UDVA and CDVA were 0.15± 0.11 and 0.10± 0.10 logMAR, respectively. Binocular contrast sensitivity function both under photopic and mesopic conditions was good. The mean absolute IOL rotation was 2.52± 2.59 degrees with 98.56% of eyes having a rotation of less than 10 degrees.
Conclusion: Bilateral implantation of an aspheric toric monofocal IOL with a double C-loop haptic design in cataract patients with corneal astigmatism provides good visual and refractive outcomes up to 1-year post-surgery.

Rotational Stability After Implantation of an Aspheric Toric Monofocal IOL with a Double C-Loop Haptic Design in Normal and High Axial Lengths

Purpose: This study focuses on the analysis of the rotational stability after implantation of a biconvex aspheric toric monofocal intraocular lens (IOL) with a double C-loop haptic-design in eyes with normal versus high axial lengths.
Methods: This was a prospective comparative study including 150 eyes with corneal astigmatism (≥ 0.75D) implanted with the biconvex aspheric monofocal PODEYE toric IOL and assessed up to 1 year after surgery. From these 150 eyes, 81 and 69 eyes belonged to normal (≤ 23.9mm) and high (≥ 24mm) axial lengths, respectively. About 4– 6 and 11– 13 months postoperatively, refraction and rotational stability of the PODEYE toric IOL were evaluated to find possible differences among both groups.
Results: At the last postoperative-visit, 74.07% of eyes were within ± 0.50D and 95.06% of eyes were within ± 1.00D of the target-refraction for the normal group and 92.75% and 100% for the high axial length group, respectively. For the normal group, 77.78% and 96.30% of eyes had a postoperative refractive cylinder of ≤ 0.50D and ≤ 1.00D, respectively; and for the high axial length group these percentages were 84.05% and 100%, respectively. About 81.84% of eyes in the normal and 94.20% of eyes in the high axial length group were < 5 degrees of the intended axis. The mean IOL rotation was 2.93 ± 2.48 degrees in the normal axial length group, and 2.17 ± 1.73 degrees in the high axial length group. Based on the mixed ANOVA results, there is no evidence of a significant difference in IOL rotation over time, nor is there evidence that axial length plays a statistically significant role in rotational stability between 4– 6 and 11– 13 months postsurgery. Both groups exhibited similar and stable rotation outcomes.
Conclusion: Our results reveal that the biconvex aspheric monofocal PODEYE toric IOL with double C-loop haptic-design shows excellent rotational stability without significant differences between eyes with normal and high axial lengths.

Rotational stability and clinical outcomes after implantation of a new monofocal toric intraocular lens with double C-loop design

Purpose: To investigate rotational stability and visual outcomes of patients unilaterally or bilaterally implanted with a new monofocal toric intraocular lens (IOL).

Setting: Ophthalmology service, clinique Beausoleil, avenue de Lodève, Montpellier.

Design: Single-center retrospective study.

Methods: This study included patients who underwent routine cataract surgery with the PODEYE toric (BVI/PhysIOL SA, Liège, Belgium) IOL using the ZEISS CALLISTO eye®. Biometry and keratometry data, refractive outcomes, rotational stability, and astigmatism correction were recorded. IOL rotation was evaluated using an image analysis technique. Postoperative assessments were performed at 1 week, 1 month, and 4 to 6 months after surgery.

Results: Clinical outcomes of 102 patients (136 eyes) were analyzed. Patients had a mean age of 74 years. Of the included eyes, 25% had an axial length greater than 24.5 mm. Median postoperative IOL rotation from baseline (surgery) was 2̊. With the exception of one outlier (15̊ rotation), IOL rotation was ≤ 6̊ (1 month) and ≤ 10̊ (4–6 months) in 100% of the eyes. No surgical IOL re-positioning was required. Median postoperative corrected distance visual acuity was −0.08 logMAR, and median postoperative subjective cylinder was between 0.25 and 0.50 D.

Conclusion: The PODEYE toric IOL showed high rotational stability, allowing for correction of corneal astigmatism during cataract surgery.

Stabilité rotationnelle et résultats cliniques après l’implantation d’une nouvelle lentille intraoculaire torique monofocale à anse en double C

Objectif: Étudier la stabilité rotationnelle et les résultats visuels de patients implantés avec une nouvelle lentille intraoculaire torique monofocale.

Cadre: Service d’ophtalmologie, clinique Beausoleil, avenue de Lodève, Montpellier.

Conception: Étude rétrospective monocentrique.

Méthodes: Inclusion de patients opérés de cataracte avec implantation du PODEYE torique (BVI/PhysIOL SA, Liège, Belgique) à l’aide du ZEISS CALLISTO eye®. Les données biométriques et kératométriques, les résultats réfractifs, la stabilité rotationnelle et la correction de l’astigmatisme ont été collectés. La rotation de l’IOL a été évaluée à l’aide d’une technique d’analyse d’image. Les évaluations ont été effectuées à 1 semaine, 1 mois et 4 à 6 mois après l’opération.

Résultats: Cent deux patients (136 yeux) ont été analysés. Les patients avaient en moyenne 74 ans, dont 25 % avec une longueur axiale supérieure à 24,5 mm. La rotation médiane postopératoire était de 2̊. À l’exception d’un cas (rotation de 15̊), la rotation était ≤ 6̊ (1 mois) et ≤ 10̊ (4–6 mois) dans 100 % des yeux. Aucun implant n’a été repositionné. En postopératoire, l’acuité visuelle de loin corrigée médiane était de −0,08 logMAR, et le cylindre subjectif médian entre 0,25 et 0,50 D.

Conclusion: Le PODEYE torique a montré une grande stabilité rotationnelle, permettant la correction de l’astigmatisme cornéen au décours de la chirurgie de la cataracte.

Visual and Refractive Outcomes After Bilateral Implantation of a Biconvex Aspheric Toric Monofocal Intraocular with a Double C-Loop Haptic Design

Purpose: To show the visual and refractive outcomes in cataract patients with corneal astigmatism when bilaterally implanted with a biconvex aspheric toric monofocal intraocular lens (IOL) with a double C-loop haptic-design.
Methods: Forty-seven cataract patients (94 eyes) with corneal astigmatism (≥ 0.75D) were implanted with the monofocal PODEYE toric IOL and assessed for 4– 6 months post-surgery. Measurements included monocular and binocular uncorrected-distance visual acuity (UDVA) and corrected-distance visual acuity (CDVA), under both photopic and mesopic lighting conditions. Refraction, photopic and mesopic contrast sensitivity (with and without glare), and rotational stability were also recorded at the last postoperative visit.
Results: At 4– 6 months, 78.2% and 98.9% of eyes were within ± 0.50D and ± 1.00D of the target refraction, respectively. The mean spherical equivalent and refractive cylinder values were 0.09± 0.35D and − 0.36± 0.35D, respectively. 76.5% and 98.8% of eyes presented a postoperative refractive cylinder of ≤ 0.50D and ≤ 1.00D, respectively. 91.5% and 100% of patients had a binocular UDVA and CDVA of ≥ 20/25, respectively. The mean binocular UDVA and CDVA were 0.02± 0.08 and − 0.02± 0.07 logMAR, respectively. Under mesopic conditions, 78.7% and 83.0% of patients presented a binocular UDVA and CDVA ≥ 20/32, respectively. The mean binocular UDVA and CDVA were 0.15± 0.11 and 0.12± 0.11 logMAR, respectively. The patients showed good contrast sensitivity under photopic and mesopic conditions. The mean absolute IOL rotation was 1.22± 2.21 degrees with 97.87% of eyes having a rotation of < 10 degrees.
Conclusion: This study shows good visual and refractive outcomes for the PODEYE toric IOL when implanted bilaterally in cataract patients with corneal astigmatism.

Visual and Refractive Outcomes of a New Hydrophobic Trifocal Toric Intraocular Lens

Purpose: To evaluate the visual outcomes and efficacy of astigmatism correction using a new hydrophobic trifocal toric intraocular lens (IOL).
Methods: This study involved 62 eyes implanted with the FineVision HP Toric IOL. The visual and refractive outcomes were assessed preoperatively and 6 weeks after the surgery. Specifically, monocular uncorrected distance visual acuity (UDVA), corrected distance visual acuity (CDVA), uncorrected intermediate visual acuity (UIVA) at 80 and 60 cm and uncorrected near visual acuity at (UNVA) at 40 cm were evaluated. The rotational stability of the lens was also assessed.
Results: Sixty-one eyes (98.39%) were within ± 1.00D and 55 eyes (88.71%) were within ± 0.50 D of spherical equivalent, with a mean value of 0.09± 0.39 D. 51 (82.26%) and 61 (98.39%) eyes had a UDVA of ≥ 20/20 and ≥ 20/25, respectively, and for CDVA these values were as follows: 59 (95.16%) and 62 eyes (100%), respectively. The mean UDVA and CDVA were 0.01± 0.06 and − 0.01± 0.04logMAR, respectively. Greater than or equal to unaided 20/20 vision was achieved at 40 cm in 42 (67.74%), UIVA at 60 cm in 42 (67.74%) and 50 eyes (80.65%) at 80 cm. Those achieving ≥ 20/25 were 56 (90.32%, 40 cm), 59 (95.16%, 60 cm), and 62 eyes (100%, 80 cm). Postoperative mean values were 0.04± 0.07, 0.03± 0.07, and 0.00± 0.07logMAR for UCNVA, UIVA at 60 cm, and UIVA at 80 cm, respectively. The mean rotation of the IOL was 5.8 degrees.
Conclusion: This hydrophobic trifocal toric IOL provides good refractive outcomes with excellent visual acuity across multiple distances, providing a full range of focus.
